The Michigan Professional Licensing User System (MiPLUS) is the Department of Licensing and Regulatory Affairs, Bureau of Professional Licensing's new online licensing and regulatory database application for health and occupational professionals in Michigan.
- Applicants can now apply online, track the status of their application, and receive electronic notifications.
- Licensees are able to renew and modify their existing licensing information.

Important Notes for Using MiPLUS
- MiPLUS supports all major internet browsers including Chrome, Edge, Firefox, and Safari. If any issues occur while using one browser. We encourage you to switch to a different browser and try again.
- We recommend using a desktop or laptop computer to complete the registration and renewal process.
- Please review the videos or print out the instructions on "How to Register an Account in MiPLUS" and "Renew Your License" before calling or emailing as they might answer your questions.
- You must register an account in the new MIPLUS licensing system BEFORE you try to renew your license.
- When registering an account please use the name exactly as it appears on your current license. This will allow the system to find your current license and avoid having to contact the Department to link your account and your license.
- If once you register an account and login your license does not show up for renewal do not submit an application for a new license. Please contact the Department so we can link your account to your license.
- The application and renewal process requires payment with a credit or debit card - Visa, Mastercard, American Express, or Discover.
- While the average time to receive a new license is 4-6 weeks and could be sooner if all required documents are received with the application, renewals are generally completed within a few days.
- If you receive an error that says either your email address or user name are already in use by an existing Citizen Access account you can still use the same email address or user name by following the instructions found HERE .
- Changing your name as part of the renewal process WILL slow down your renewal as name change documents have to be reviewed by staff before your renewal is processed. Renew your license first and then follow the steps to “Modify a License” to change your name. While your renewal license will have your old name, when you “Modify Your License” a free copy of a license with your new name will be emailed to you. (hard copies are $10)
- Businesses (for example real estate brokers, pharmacies, barber shops and cosmetology shops) received a PIN letter in the mail. Please wait to receive your PIN letter and instructions before setting up your MiPLUS Account. For instructions on using your PIN for pharmacies click HERE and for other businesses click HERE .
- To change your name and/or address on your license, to order a reprint, add a specialty to your license or file for relicensure, log into MiPLUS then select “Modification” next to your license number.
- To apply for relicensure, log into MiPLUS then select "Modification" next to your license number and then select "Relicensure."

How to Change Your Name on Nurse Practitioner Licensing
Do you hear wedding bells in your future? Or, maybe you are more like me and the wedding bells rang long ago but you have some sort of pathological aversion to paperwork and embarrassingly have yet to change your last name. Well, I overcame my deepest paper pushing fears this winter and have finally completed the name change process including the complete array of nurse practitioner certifications and licenses.
Since I have just tackled this dreaded task, which by the way wasn’t that bad, I thought I should share a brief synopsis of the process to hopefully save you some headaches in the name change transition. Here’s a brief how-to guide for changing your name as a nurse practitioner .
Step 1: Grab a book or magazine, a cup of coffee, some Purell, your social security card and marriage certificate. Head to the local social security office. Take a number and have a seat in the most uncomfortable plastic chair you can find (don’t worry, they are everywhere). Wait an hour or so until your number is called. Get your name changed on your social security card by the first employee who seems almost motivated enough to help you.
Step 2: Repeat the above at the Department of Licensing changing your last name on your driver’s license. It is recommended that you refill your Purell first. Also, you might consider running a brush through your hair and wearing a shirt you like. No, you don’t have to remove your slippers- these won’t show in your new driver’s license photo.
Step 3: Don’t you hate your new signature? When I attempt to sign my new last name it looks like I am trying to re-learn how to write in cursive. At this point, you should notify your employer you are changing your name. They will need to get you the appropriate Medicare paperwork so you can continue to get paid by insurers (important especially since you probably just dropped some serious cash paying for that wedding!) under your new name.
Step 4: Armed with your new documents and marriage certificate, visit your state’s board of nursing website. No Purell, hairbrush or cute shirt necessary, you won’t have to leave the house for this one. You will need to change your name on both your RN and advanced practice nursing license. You may be able to do this online or by printing the necessary paperwork and mailing it to the board of nursing. This typically must be done within a certain number of days of changing your name on your social security card and driver’s license. Based on my experience, however they won’t send you to jail if you wait, oh six months or more.
Step 5: Tired of paperwork yet? Now change your name on your nurse practitioner national certification , either through the AANP or ANCC. This can be done easily online.
Step 6: You’re getting close… Visit the NPI website, login and change your name in the National Provider Identification system . Forget your username, password and all the answers to your security questions? I did too. I mean, who uses “What is your shoe size?” as a security question. Obviously it depends on the pair of shoes. In the future I recommend questions like “What is the size of your favorite neon pink Asics running sneakers that have a hole in the side because you thought your puppy was over the shoe chewing stage?” Don’t hesitate to call the NPI 1-800 number for assistance. You won’t be on hold long and they are very helpful.
Step 7: Almost there… Login to your DEA account online and change your name in the DEA system. They will send you a new DEA license via snail mail.
Finally! Once you have completed these steps you are ready to practice under your new last name. Fortunately for me, my signature is messy enough that although I changed my last name, I did not find it necessary to alter my autograph.

To file a change of name: Please complete our online Name Change Form and attach supporting documentation. Once you are licensed, Education Law requires that you notify the Department of any change in your name within 30 days of the change.
If you are in a current application for Exam, Reinstatement or Endorsement, please send a message through your Nurse Portal Message Center with your attached name change legal document. There is no fee for a name change if you are in an active application.
Any changes must be done in accordance with the board's requirements. Go to the nursing board's website and look for a "name change" tab or similar link. The instructions will tell you what you need to do to change the name on your license.
You will need to change your name on both your RN and advanced practice nursing license. You may be able to do this online or by printing the necessary paperwork and mailing it to the board of nursing.
1. How do I change my address or name on my license? You may submit name and address changes to the Board by completing the name or address change form. Click on the info tab at the top of the Maryland Board of Nursing (MBON) homepage. The forms are located under "Board Services and Links.” Complete
If you are a licensed California registered nurse residing and working outside of California, be sure to contact the California Franchise Tax Board (CFTB) and provide your change in address of record to ensure receipt of any notices they might send you. You may contact the CFTB at (800) 852-5711.
If your name on Nursys matches the name on the state board of nursing's website, you will need to contact the state board of nursing directly to request a name change. NCSBN's Nursys website is a primary source equivalent database.
If you are currently licensed, enter your New York State license number. DO NOT USE THIS FORM IF YOU NEED TO CHANGE YOUR ADDRESS ONLY. TO ONLY CHANGE YOUR ADDRESS, SUBMIT A CONTACT US FORM AT https://eservices.nysed.gov/professions/contact-us/#/.
The Texas Administrative Code 217.7 states that all nurses/applicants for licensure shall notify the board in writing within ten days of a change of name and or address by submitting a legal document reflecting this name change and/or providing the new address with his or her license number.
